Accident facts and aircraft context

On Saturday, 9 January 2021, at 13:36, a Cessna 560 Citation V (registration N3RB) was involved in a fatal accident approximately 23 km southeast of Pine Grove, Oregon, United States.

The aircraft had departed from Troutdale Airport, Oregon (TTD/KTTD) and was en route to Boise Air Terminal, Idaho (BOI/KBOI).

Purpose of the flight

Single-pilot private flight in a Cessna Citation V.

Flight sequence and key events

Tragically, the aircraft was destroyed on impact, and the sole pilot on board lost his life.

Shortly after departure, while communicating with Portland Approach and later Seattle Center, the pilot experienced difficulty following air traffic control instructions, particularly regarding assigned headings and radio frequency changes.

The aircraft was cleared to climb to FL370, but it only reached a maximum altitude of 31,000 feet at 13:28:45.

After reaching that altitude, the aircraft began descending and spent the next eight minutes in a tight spiraling right turn within a 1-mile radius.

The final ADS-B signal was recorded at 3,800 feet, moments before the aircraft impacted terrain in the Mutton Mountain Range at an elevation of approximately 3,600 feet.

FAA records showed that the pilot did not possess the required type rating for the Citation 560.

Although he had undergone training in late 2020, instructors reported that his performance was not sufficient for certification or a single-pilot exemption.

Reports from the training facility and acquaintances indicated that he struggled in high-workload situations, had difficulty managing the aircraft’s upgraded avionics, and preferred manual flying over autopilot use.

Historical flight data strongly suggested that this may have been his first solo flight in the aircraft, which likely contributed significantly to the accident

Investigation findings

Investigations later revealed serious concerns regarding the pilot’s qualifications and familiarity with the aircraft.

The NTSB final investigation found that N3RB lost control because of pilot incapacitation for reasons that could not be determined. The 72-year-old pilot had difficulty maintaining assigned headings and altitudes early in the flight, the aircraft climbed to about 31,000 feet, then entered an approximately eight-minute spiral descent before impact. Investigators found the flightpath during the descent was consistent with the pilot no longer manipulating the controls. The NTSB also examined the pilot's medical history and unreported conditions and medications, but could not determine the exact reason for the incapacitation.

The Citation accident developed after a difficult climb and communications sequence into a prolonged spiral descent. The NTSB ultimately determined that pilot incapacitation caused the loss of control, but the investigation could not establish why the pilot became incapacitated. That distinction is important: the incapacitation is the final finding; its medical mechanism remains undetermined.
