A Barrel Roll and Seven More Minutes

The left engine failed first, on schedule with nothing especially dramatic about it — the kind of failure a twin-engine turboprop crew trains for and generally handles without much drama. What made 7 December 2016 different was what happened as Pakistan International Airlines Flight 661's crew worked through their standard engine-failure procedure: the propeller, instead of feathering as commanded, began behaving unpredictably. The aircraft performed a full 360-degree barrel roll and lost thousands of feet of altitude before the crew regained control. They flew on for another seven minutes. Then they lost control a second time, and this time they didn't recover it.

Thirty-Nine Passengers, a Singer, an Engineer

The ATR 42-500, registration AP-BHO, had been flying its sixth leg of the day, a short domestic hop from Chitral to Islamabad carrying 42 passengers and five crew, including three pilots and a mechanic. Among the passengers was Junaid Jamshed — once one of Pakistan's most famous pop singers, later a well-known Islamic evangelist — traveling with his wife. Both died with everyone else aboard when the aircraft went down near Havelian, roughly 50 kilometres short of Islamabad.

A Blade With a Known Problem

It took investigators nearly four years to fully untangle what actually happened inside that left engine, and the answer turned out to be more than a single broken part. A power turbine blade in the No. 1 engine had a distress mode — a known failure pattern on this specific engine family, one manufacturers had been aware of and working to address since 2007. Pratt & Whitney Canada issued a new blade design in October 2015, and updated the engine maintenance manual the following May with replacement criteria for both old and new blade designs. PIA's own engine, it turned out, had been removed from a different ATR aircraft and installed on this one — and its maintenance history hadn't caught the blade's degradation before it began contaminating the engine's oil system mid-flight.

Three Faults, Not One

The final investigation report, released in November 2020, identified three separate latent faults working together: the fractured power turbine blade, a broken pin, and contamination — probably pre-existing — inside a propeller valve. As the damaged blade degraded in flight, it worsened vibration in the left engine and contaminated the lubrication system badly enough that the electronic engine controller automatically shut the engine down. The crew tried to restart it three times. Each attempt failed.

The Propeller That Wouldn't Feather

With the engine down, the crew needed the left propeller to feather — turning its blades edge-on to the airflow to minimize drag, the standard procedure that lets a twin-engine aircraft fly safely on one engine. Instead, contamination inside the propeller valve mechanism meant the propeller went out of feather entirely, its blades swinging toward a low-pitch position that created enormous drag rather than reducing it — investigators estimated roughly 2,000 pounds of force fighting the aircraft's forward motion. That uncommanded, unfeathered propeller is what triggered the barrel roll and the sharp loss of altitude the crew fought through the first time.

Forty-Two Minutes, Two Losses of Control

The flight lasted 42 minutes from takeoff to impact. The crew's response to the first loss of control — recovering from a full roll and thousands of feet of unplanned descent — reflected real skill under conditions almost no training scenario fully replicates. But the underlying mechanical problem, three separate faults compounding each other inside a single engine, never resolved. Seven minutes after regaining control the first time, the aircraft entered a second, final loss of control it never came back from, crashing into mountainous terrain near Havelian.
