(Fatal) Detached Cabin Door Strikes Propeller After Takeoff
"I Simply Missed Locking It"
Weeks after the crash, once he was well enough to talk it through, the pilot told a flight instructor something simple and honest: he'd just missed locking the copilot's door before departure. That single unlatched door — on 2 August 2010, in a Velocity Super XL RG-5 climbing out of Montgomery Field in San Diego — was enough to bring an airplane down onto a golf course and kill two of the five people aboard.
A Door, Then a Vibration
A witness at the airport watched the right passenger door swing open during taxi and stay open through takeoff. The pilot himself later told investigators he had no memory of it being open at that point — his recollection only picks up after the airplane was airborne, when he noticed the door and radioed the tower that he'd be returning to land. During the turn back, he felt a vibration. He assumed, correctly as it turned out, that the door had come off the airframe entirely and struck something behind the cabin.
Pusher Configuration, Rear-Mounted Risk
The Velocity XL series is a canard-configured, all-composite kit aircraft with a distinctive layout: a small forward wing, and the engine and propeller mounted at the rear of the fuselage in a pusher configuration, spinning behind the cabin rather than in front of it. It's a design that offers real advantages — better forward visibility, favorable aerodynamics, genuine cross-country speed — but it also means anything that comes loose from the cabin has a direct path toward the engine and propeller assembly sitting immediately behind it, rather than falling harmlessly away from a nose-mounted engine the way it would on a conventional aircraft.
What Investigators Found a Mile Away
The door itself was recovered about a mile west-southwest of the crash site — a measure of how violently it separated and how far it traveled once it left the airframe. Examination showed the door's locking mechanism was intact and undamaged; there was nothing wrong with the latch itself. What investigators found instead were black rubber signature marks on the lower forward section of the door, consistent with contact with the engine's drive belt — which was found separated from the engine — and matching rubber impact marks on all three propeller blades. The door hadn't just banged against the fuselage. It had gone directly into the engine's moving parts.
A Belt, Not Just a Blade
The loss of engine power that followed wasn't from a bent propeller alone — the NTSB's investigation found the door's impact damaged the drive belt itself, the component transmitting power from the engine to the propeller in this pusher configuration. With that connection compromised, the aircraft lost thrust during exactly the moment — a turn back toward the airport at low altitude — when a pilot has the least margin to recover from any kind of power loss. The pilot made a forced landing at Admiral Baker Golf Course rather than trying to stretch a glide back to the runway.
An Ordinary Mistake, an Unusual Airplane
The NTSB's final report placed the cause plainly: the pilot's failure to close and secure the right passenger door, leading to its in-flight separation and subsequent collision with the rear-mounted engine assembly. It's a mistake that happens on conventional aircraft too — doors come open in flight more often than most passengers realize, and pilots train specifically for it, because on a normal tractor-configuration airplane an open door is loud and startling but rarely dangerous.

Kit-built aircraft like the Velocity XL series don't always share the same door hardware conventions as factory-certified aircraft, and that's worth understanding rather than glossing over.
The Aftermath on the Golf Course
Admiral Baker Golf Course, where the aircraft came down, sits close enough to Montgomery Field that an emergency return under partial power can reasonably reach it — which is likely exactly why the pilot chose it once he recognized he'd lost engine power during the turn back.
