Avianca 052: Fuel Exhaustion and a Fatal Failure to Communicate

Avianca Flight 052 was approaching New York on January 25, 1990, after a long flight complicated by poor weather, multiple holding patterns and diminishing fuel. The Boeing 707-321B eventually lost power to all four engines and crashed at Cove Neck on Long Island. Seventy-three of the 158 people aboard died.

The flight had departed Bogotá and stopped at Medellín before continuing toward John F. Kennedy International Airport. The crew had planned fuel for the trip, reserves, an alternate and holding. But the weather around the northeastern United States caused repeated delays.

The aircraft was placed into three holding patterns. Together they lasted about 77 minutes. During the third hold near CAMRN, the crew told controllers they could hold for only about five more minutes and that Boston, the planned alternate, was no longer reachable because of fuel.

That transmission was a critical moment. The crew was already close to exhausting the margin needed to divert.

The NTSB's final report found that the flight crew had not adequately managed the airplane's fuel load and had failed to communicate an emergency fuel situation clearly enough before fuel exhaustion occurred. The investigation also identified contributing issues involving dispatch support, traffic-flow management and terminology used between pilots and controllers.

The communications problem was not simply that the crew never mentioned fuel. They did. The difficulty was that the words used did not communicate the level of emergency that the controllers needed to understand.

The NTSB transcript records the exchange during the third hold. The crew said they could hold for about five minutes and could not reach Boston because they would run out of fuel. The controller then worked to obtain a clearance toward Kennedy.

The aircraft subsequently began its approach to JFK. A missed approach followed after windshear affected the first attempt. The airplane then attempted to return for another approach.

By this point, the fuel margin had disappeared. All four engines lost power. The aircraft crashed approximately 16 miles from JFK in Cove Neck.

The accident became one of the most influential examples of fuel-emergency communication in commercial aviation. It demonstrated that low fuel and emergency fuel are not interchangeable operational concepts when a controller must decide whether an aircraft needs immediate priority.

The NTSB report also emphasized the role of the dispatch system, ATC traffic management and standardized terminology. The accident was therefore not reduced to a single cockpit decision. It was a chain involving fuel planning, weather, holding, communication and traffic flow.
